To obtain Multi-Family/Dual Residency “Non-Cedar Hill Residents”

Parent/Legal Guardian must:

Complete the Dual Residency Forms provided by the campus/administration.

Statement of Multi-Family/Dual Residence Parent/Guardian Application for Multi-Family/Dual Residency Resident Application for Multi-Family/Dual Residency

• Appear with the Cedar Hill Resident whose name appears on the water, gas, or electric bill. This must be completed before the appointed school notary for the purpose of signing required statements pertaining to Dual Residency.
  
  
• Provide a current and valid Texas Driver’s License or Texas Identification Card. (The Driver’s License or Texas ID MUST contain the address listed on the “Statement of Multi-Family/Dual Residence” form).

Cedar Hill Resident must:

• Present the current (for month of enrollment) water, gas, or electric bill with his/her name, address and date clearly imprinted on the bill.
  
  
• Current and valid Texas Driver’s License or Texas Identification Card (The Driver’s License or Texas ID MUST contain the address listed “Statement of Multi-Family/Dual Residence” form).
  
  

The child’s presence in the Cedar Hill Independent School District is not for the primary purpose of participation in extracurricular activities.

Providing false information in order to obtain enrollment in a public school is illegal and carries criminal penalties. [TEC 25.001(h) and Penal Code 37.01]

*MULTI-FAMILY/DUAL RESIDENCY IS VALID ONLY FOR THE CURRENT YEAR. THEY WILL NEED TO BE RENEWED YEARLY
